Ponte Vedra High School Sports Physicals for the 2012-13 school year will be held at the high school on Sunday, June 3, 2012.

Physicals are open to any students intending to tryout or participate with athletic teams at PVHS in the 2012-13 school year, including current 8th graders.

The $20 cost goes directly to PVHS athletics and provides the standard health evaluation as well as an orthopedic evaluation with representatives of Heekin Orthopaedic Associates.

This physical will clear the student for athletic participation at the high school for the entire 2012-13 school year.

Please arrive at PVHS on the schedule listed below.

Directional signs will be posted in the blue parking lot behind the gymnasium.

To expedite your physical please bring with you a completed and signed

FHSAA Preparticipation Physical form and a completed and signed FHSAA Parental Consent form.
